Body Feminization Basics

For many transfeminine patients, top surgery and bottom surgery are the most commonly discussed procedures to complete their transition. Not every transgender women may desire a more feminine shape, such as enhanced curves or a larger buttock, but for the patients that do, Dr. Stiller is here to help. Many patients that see Dr. Stiller have been on long term estrogen therapy, although it is not required. Estrogen therapy can be helpful for many patients in enhancing their feminine shape. However, the final benefits for feminizing changes are usually seen at 5 years. This is when most patients will further their transitional journey and choose body feminization surgery.

This is done through the process of liposuction and redistribution to the hips and buttock, creating and enhancing your curves.  

Androgenic and Estrogenic Body Forms

The key to successful feminization for transgender patients is a thorough understanding of the differences between the male and female physique. For cisgender males, fat distribution tends to be more in the abdomen and flanks. This gives the body a more boxier and less curvaceous shape as well as the gluteal form having a 'flatness' or concavity. Cisgender females tend to carry their weight in the hips, buttocks and thighs due to estrogenic puberty giving them a greater curve to their hips and a fuller round gluteal shape. For transgender females, who did not experience an estrogenic puberty, the fat distribution can result in a masculine-looking body contour causing a great amount of gender dysphoria. However, with the help of liposuction and fat redistribution, Dr. Stiller can help you a achieve a more feminine silhouette.

Liposuction

The body feminization process begins with your consultation. During your consult with Dr. Stiller, you will discuss the areas that are most bothersome and trigger your gender dysphoria. He will review your options and discuss ways to enhance your figure through liposuction and redistribution of the fat. To feminize the body, Dr. Stiller will most commonly perform liposuction to the abdominal area and flanks to help create a narrower waistline and enhance the shape of your upper buttock. Some patients may desire liposuction alone, without buttock enhancement, and that is perfectly okay. You will discuss your goals with Dr. Stiller to best achieve what you are looking for.  

It is important to note patients choosing liposuction are usually around their normal weight or slightly above. Significantly overweight patients or patients with a significant amount of loose skin are generally not good candidates for liposuction alone.

Quality of skin is another important factor in determining if Liposuction is right for you. Thin skin, large amounts of excess skin, or skin damaged by stretch marks can make it difficult for the skin to contract after fat tissue is removed, which can leave the area looking more deflated rather than tight and contoured.

Brazilian Butt Lift

While the removal of fat from the waist area can help to feminize the torso, the re-introduction of fat to the buttock can give an even greater result to better accentuate your new female figure. A Brazilian butt lift involves harvesting excess fat from areas such as the abdomen, flanks, etc. and placing it to areas of the buttock, most commonly outer portion of buttock and thigh, to achieve a curvy look that unmistakably feminine. With the use of your own natural fat, your buttock can look and feel more natural giving you the look you always desired.

Recovery

Both liposuction and BBL procedures are outpatient procedures, so after a short period of recovery time, you will go home the same day. Recovery after liposuction alone can take 2-4 weeks for swelling and discomfort to subside. Combined with a Brazilian Butt Lift, recovery can take a bit longer due to the need to refrain from sitting, lying down, or putting direct pressure on the backside for 6 weeks because the new fat cells can be easily damaged.  If you must sit down, it is important to use a Brazilian Butt Lift pillow, or donut pillow, to go under your thighs and alleviate pressure from your buttock. After both procedures, you will be in a compression garment for 4 weeks total. Most patients take about 1-2 weeks off work; however, some may take longer or return sooner depending on the physical demands of their job.
